We are committed to supporting the members of the P2 collaborative in providing the Western New York community with ongoing information for healthy living.

The P2 Collaborative (Pursuing Perfection) is a community-wide collaborative that has 170 partners – community organizations, providers, and health plans – who work together to improve the quality of health care and the health of people in Western New York.

Over the years unhealthy lifestyles have contributed to an increase in individuals living with chronic diseases. Lack of physical activity coupled with an abundant food supply has lead to unprecedented rates of obesity, obesity-related diseases, and other chronic illnesses. As our population ages, particularly in Western New York, the situation will only get worse.

To move toward reversing this unsustainable trend, the P2 Collaborative is dedicated to putting our community on the road to better health and wellness by:

  • Educating and motivating people in Western New York to make lifestyle changes to prevent illness and help those who are ill, become well again.
  • Ensuring that our community understands that they are partners in their own health care, and that there are guidelines based on expert opinions that they and their physicians can use together to manage their care.
  • Ensuring that our community has access to quality health care and, when members of our community become ill, they receive the most effective and up to date care.


The P2 Collaborative is committed to the good health of the Western New York community. Through the collaborative promotion of healthy lifestyles, P2 works to improve the individual quality of life, as well as reduce the cost of health care -- so the future of Western New York is a healthy one. What Western New Yorkers Want for the Future of Health Care in WNY
Long before the national debate on health care began, the Community Health Foundation of Western & Central New York (CHFWCNY) began working to give the Western New York community a voice on the issue by asking: What do Western New Yorkers Want for the Future of Health Care?
Getting the community’s perspective and hearing what the users of the health care system wanted out of any future design were of utmost importance to CHFWCNY and its partners: The John R. Oishei Foundation, P2 Collaborative of Western New York and the University at Buffalo Regional Institute.   The work began in 2007.  Since then over 110 facilitated conversations with the community were held where more than 1,700 Western New Yorkers answered the question.  Their responses were compiled and the top five priorities were identified.  These findings were presented at a public launch on July 28, 2009.
